The Sweethearts Foundation works with qualified occupational therapists and mobility partners to ensure that every wheelchair provided is clinically prescribed and appropriate for the individual’s needs.

Wheelchairs are not distributed without a professional assessment.
Each request is carefully assessed to ensure the correct mobility solution is provided.
Who can apply
Wheelchair requests may be submitted for individuals who:

Require a wheelchair for long-term mobility

Have been assessed or are willing to be assessed by a qualified occupational therapist
-
Do not have access to other funding options for mobility equipment
Requests may be submitted by:

A wheelchair request form is submitted with the required information.
The beneficiary is assessed by a qualified occupational therapist.
A suitable wheelchair is prescribed based on the individual’s needs.
The Foundation works to secure funding for the wheelchair.
Once funding is secured, the wheelchair is manufactured and a wheelchair handover is planned.

Submit a Wheelchair Request

Download the Required Forms
Download and complete the required application documents. These documents must be completed in full and signed by the appropriate parties.

Complete and Certify the Documents
All required forms must be completed and signed. Certain documents must also be certified by a Commissioner of Oaths. Please ensure that all information is accurate and that all required signatures are included.

Get supporting documentation ready
During your application, you will need to upload supporting documents. These help us assess the beneficiary’s needs and process the request correctly.
Please have clear digital copies of the following documents ready before you begin:

Beneficiary’s ID, Birth Certificate, or Passport

Guardian’s ID, Birth Certificate, or Passport (if applicable)

Medical Aid Letter (if applicable)

Proof of Income or Financial Support (if applicable)

Any medical reports or letters from a doctor that support this request (if available)

Photos of the beneficiary to assist with the assessment

Submit Your Request
Once the documents have been completed and certified, you may submit your wheelchair request through our online request form. During submission, you will be required to upload the completed documentation.
Important
Requests submitted without the required completed documentation will not be processed. Please ensure all forms are completed, signed, and certified before submitting your request.
