A sworn statement confirming the personal and household circumstances of the beneficiary. This document helps The Sweethearts Foundation verify information provided in the request and ensures that assistance is allocated fairly and responsibly. Incomplete or unsigned affidavits will result in the request being considered invalid.
This form gives The Sweethearts Foundation permission to process personal information in accordance with the Protection of Personal Information Act (POPIA). It also allows applicants to indicate whether they consent to photographs or stories being used for awareness or fundraising purposes.
This form is completed by a qualified healthcare professional, such as an Occupational Therapist or Physiotherapist. It provides the clinical information and measurements needed to determine the most suitable wheelchair or mobility device for the beneficiary.
The EEA1 form collects demographic information required for reporting and compliance purposes under South African regulations. This information helps The Sweethearts Foundation monitor equity, track impact, and ensure responsible programme reporting.
This document provides an overview of the financial circumstances of the beneficiary and their household. It helps The Sweethearts Foundation understand the level of financial need and ensures that support is directed to those who are unable to fund a wheelchair themselves.
All forms must be completed in full and signed where required. Incomplete forms or missing supporting documents will result in the request not being processed.
